Description

1. Introduction:



A. This is a combined synopsis/solicitation for commercial items prepared and supplemented with additional information included in this notice. This announcement constitutes the only solicitation; offers are being requested.



B. This solicitation is set aside for Service Disabled Veteran Owned Small Business. The applicable NAICS code is 337110 and the Small Business Size Standard is 750 employees.



C. There will be a site visit offered to take field measurements of the space on September 21st at 1000 ET. Contractors are to meet in the lobby of the main hospital.



2. Responding to this solicitation. Offeror must submit its offer via Email to alison.klein@va.gov. Offeror must provide the following information with its offer:



A. Legal entity name, address, SAM UEI of your company; firm must be listed in https://veterans.certify.sba.gov/ as a SDVOSB at time of offer.



B. Information that demonstrates:



(1) Firm can provide was is being requested i.e. cutsheets and specs for the cabinets, countertop, facet, shelves and eyewash station.



C. Price Schedule



D. Proof of insurance



E. VA NOTICE OF LIMITATIONS ON SUBCONTRACTING—CERTIFICATE OF COMPLIANCE
