STATEMENT OF WORK
EDWARD LIFESCIENCES HEMOSPHERE
Brand Name Only
PURPOSE
The Martinsburg VA Medical Center requires the replacement of their hemodynamic advanced monitoring unit. This unit is used in the intensive care unit (ICU) for critically ill patients to guide the use of vasoactive medications, resuscitation efforts and intra/post operatively for complex vascular surgeries. Procuring this device will help increase patient recovery from surgeries.
HOURS OF OPERATION
Normal hours of coverage are Monday through Friday, 7:00 AM to 4:30 PM Central Standard Time excluding national holidays. All services shall be performed during normal hours unless otherwise scheduled with the COTR or his/her designee. Contractor shall always be professional and shall take all necessary precautions to maintain a safe environment for the occupants. Contractor shall fully minimize noise possible while working in all buildings.
Note: Hardware/software update/upgrade installations, where applicable, will be scheduled and performed outside normal hours of coverage at no additional charge to the Government (unless it would be detrimental to equipment up-time; to be determined by the COTR). Federal Holidays observed by VA are: New Year s Day, Labor Day, Martin Luther King Day, Columbus Day, Presidents Day, Veterans Day, Memorial Day, Thanksgiving Day, Independence Day, Christmas Day, Juneteenth Day
SELECTION CRITERIA

Two (2) Hemosphere advanced monitoring system (HEMAQSR2) smart recovery with clearsight includes but not limited to PRESSURE CABLE FOR ACUMEN IQ / FLOTRAC, HPI, AND ACUMEN ANALYTICS kit Contains: HEM1, HEMBAT10, HEMCSM10, PC2K, EVHRS, 2 X HEMPSC100, HEMTOM10, HEMDPT1000, 194371001, Acumen Analytics
Two (2) Pressure controller cover (PCCVR) includes but not limited to HEMAQSR2, HEMCSMUPG, EV1000CS, and EVNIUPG-US kits
Two (2) Hemopshere roll stand
Two (2) Hemosphere 360 program 24mo Plan
This should include software updates and technology expansion upgrades to the equipment software
Covers physical and liquid damage
Next day replacement
All parts, labor and shipping cost covered
Technical and product phone support
One (1) of manufacturer s warranty DELIVERABLES OR DELIVERY SCHEDULE
The contractor shall deliver and install the product. After contacting COR (or COR designee) at the VA, a specific time will be scheduled for the delivery of the product and the installation within 60 days of award. All quantities are listed above that should be delivered. They shall be delivered to the following address: Attn: ICU Martinsburg VA Medical Center Building 500, warehouse 510 Butler Ave. Martinsburg, WV, 25405.
